Higher cancer detection with fewer recalls

Digital breast tomosynthesis with synthesised 2D mammography improves diagnostic performance in breast cancer screening.

This systematic review and meta-analysis examines the combination of digital breast tomosynthesis (DBT) and synthesised 2D mammography (s2D) in comparison with conventional digital mammography. The focus was on cancer detection rates, recall rates, biopsy rates and positive predictive values.

Key results

The following values were obtained for DBT+s2D:

  • Increased cancer detection rate: RR 1.35
  • Recall rate reduced: RR 0.79
  • Positive predictive value for recalls increased: PPV-1 RR 1.69
  • Biopsy rates did not differ significantly; however, PPV-2 and PPV-3 were higher with DBT + s2D.

Classification and conclusion

The study shows that modern imaging techniques can improve the efficiency of breast cancer screening programmes. The combination of higher detection rate and lower recall rate is particularly relevant, as it influences both diagnostic quality and the burden on patients. DBT + s2D can improve early breast cancer detection and reduce unnecessary recalls. The results support the benefits of modern imaging in screening programmes.
